Enrollment and Health Insurance coverage for adult children to age 26 - effective June 1, 2011

As a plan member, you may be eligible for a Fitness Reimbursement of up to $200 for an individual plan or up to $400 for a family plan. You must belong to a full fitness center for at least 4 months consecutively AND be a member of the health plan for at least 4 months to qualify.
