The dependent eligibility guidelines for the MNHG have NOT changed. In order to be considered for coverage under the subscribers plan the dependent must be a full-time student or a disabled dependent adult. See plan for further details.
As a plan member, you may be eligible for a Fitness Reimbursement of up to $150 for an individual plan or up to $300 for a family plan. You must belong to a full fitness center for at least 4 months consecutively AND be a member of the health plan for at least 4 months to qualify.
