HPHC - Employees must notify employer within
30 days and employer must notify GBS within 30 days
THP - Employees must notify employer
within 30 days and employer must notify GBS within 30 days
Please note: Fallon Senior Plan, Tufts Medicare
Preferred HMO, and Harvard First Seniority Freedom Premier
applications should be received at GBS 30 days PRIOR to the
effective date. There is NO RETROACTIVTY on these
senior plans.
